NOTE: Wednesdays are administrative days and the phones are not physically answered but there is an answering service to answer your call.

Caregivers
If you are a caregiver from a long-term care facility or residence, you may fax your requests to (480) 964-4171.  Please include the patient’s face sheet, physician or telephone order, medical Power-of-Attorney paperwork (if applicable).  Also please note the reason for the appointment on a fax coversheet with the facility’s address and phone number listed.  We will process these requests immediately and call prior to the scheduled appointment.
